Pick up a cobray m11/9 with this "Barrel extension". The hole is smaller than a .22 and the red plastics have something inside.
So what is it?

That looks like the flare launcher. It uses blanks to launch thus the small hole.

It is a 37mm Cobray flare launcher for 9mm MAC threads 3/4x10, I come into them from time to time, sold one mint in the box on GB for a little over 300 bucks recently. I initially thought it was supposed to have some kind of firing pin to strike the 37mm primer but came to discover that the pressure from the blank cartridge is enough to punch the primer, the small hole directs all of the blast right into the primer of the 37mm round. There are plenty of companies that manufacture brand new 37mm “ammo” different types of fireworks etc.

Yeah, the pressure from a blank cartridge can be under appreciated to a deadly extent. Just do a search for "Jon-Erik Hexum".
