Just installed a Hoffman Super Safety in a Colt semi-auto 9mm AR15. It won't fire in any mode because the bolt carrier won't completely close. If I take the Hoffman safety out it closes.

Any ideas?

You didnt grind the trigger enough. The bevel on the back of the trigger is what it is hanging up on. When you grind more it will start to only close in standard semi mode. then grind more until the bolt closes normally and dry functions fine with no stickiness in the bolt travel.
9mm will need sliding weight buffers. Or you will have out of battery discharge.

The problem is the weight in the rear of the bolt is curved, not flat and the lever on the Hoffman device is catching on it.

Forgo filing, grinding or milling on your 9mm bolt weight and just replace it with one of these - 9mm CARRIER WEIGHT EXTRA HEAVY

It's completely flush with the rear of the carrier channel, adds weight and completely clears the SS lever. Works perfectly in my 9mm setup.

Good luck

HKG3K IS CORRECT
you dont want the weight going forward inside the carrier
so that it sticks out on the muzzle end
you want to maintain the m16 bolt profile or else the lever will drag or break
